NHS: Inpatient wards aim to support and provide care closer to home

EAST Cambridgeshire and Fenland PCT has four inpatient wards, situated in the community hospitals at Ely and Wisbech.
At the Princess of Wales Hospital in Ely there are two wards, Wicken and Welney. The wards provide care closer to home and a variety of services to the local population.
Patients may be admitted from home at their GP's request or transferred from other hospitals. Services include access to multi-disciplinary assessment, intermediate care and rehabilitation as well as end-of-life care and palliative care.
Where possible the aim is to support patients to return home. This will include discharge planning and co-ordination of care packages for patients with complex needs
At the North Cambridgeshire Hospital in Wisbech there are also two wards, Trafford Ward and Carlisle Intermediate Care Unit.
Trafford Ward provides rehabilitation, multi-disciplinary assessment to an elderly patient group. There are two beds designated for palliative care and there are close working links with the McMillan and Hudson Palliative Care Unit, which are also situated on site and which provides day care.
Carlisle Intermediate Care Unit was opened in 2003 and provides a service for patients requiring a shorter hospital stay, usually two to six week.
Following multi-disciplinary assessment and the implementation of a treatment programme the aim is where possible to return the person to an optimum level of function. This includes rehabilitation and early supported discharge, as well as avoiding unnecessary admission to the acute hospital.
Patients whose medical condition is stable can be admitted from home or transferred from another hospital. The ward team works closely with the community teams and day rehabilitation service, which is situated at North Cambs.
